This is probably one of the most amazing fighting games I've ever seen. It includes a fat shirtless construction worker, a guy going super saiyan using Mortal Kombat style digitized sprites, and a guy in a gas mask singing "I Believe I Can Fly" while drinking a water through the gas mask as his win animation. The funny thing is that this is actually playable:
Some of the most well-known mindfuck games are gradius-style shooters, the most well known of which are Parodious and Cho Aniki. Personally I would say that the fact Parodious is just a parody of Gradius makes it explanable, but Cho Aniki is honestly too freaking weird to not mention, even if most people probably already know about it now. As you might have guessed from @Liquid Squirtle's post, a lot of the strangest games come from China. China is a hotbed for bootlegging, so a large amount of strange independent content comes out there as a result. Case in point: You've probably heard of Takeshi's challenge and it's uniqueness if you've ever seen Jontron before, so here's a little fun fact to go with this one. When asked about the game in an interview, Beat Takeshi stated that he hates all video games and actively set out to make the worst, most frustrating game as possible when making this. That explains a lot. When speaking of Jontron and weird games, an obvious example is the "Titenic" beat em' up for the NES. Titenic is actually a sprite change of another movie bootleg, which might even be a stranger choice for a movie beat em up than Titenic. You know, Killer 7 may be one of my favourite games, but God help me if you asked me to explain any of the shit going on here. I guess a teenager like me just isn't mature enough to understand the deeper messages Suda51 was trying to get across. Or maybe he's just incredibly pretentious. I'm not really sure.

To be fair, a lot of games should have a creepy (even more so than the real version) Michael Jackson delivering exposition. It'd liven up the hell out of any RTS game.
Suppose it was MJ instead of the G-Man giving exposition in Half-Life 2? I dunno, but part of me thinks the experience would be improved, somewhat.
OFF - Kinda surprised this hasn't been mentioned, then again Markiplier and probably pewdiepie have played it so I probably shouldn't be. You start off as this baseball player guy being up various ghosts and ghouls with bizarre forms, and the game takes a turn into darker, stranger territory by the third area (without spoiling anything). Middens - A collage RPG where you play as an RPG Maker edit sprite weilding a sentient revolver, traveling through the area between the materium and the void where weird shit takes form and everyone says bizarre things (think the Warp from 40k but less edgy and grimdark and more bizarre and quirky). Haven't gotten very far in this one but it also has a prequel of some kind with a similar surreal collage style.
Haven't played enough of it to say whether it's a game or a walking simulator, but it's pretty damn weird.
Oh, Middens is great! I absolutely loved that game. Have you tried their other game called "Gingiva"? It's along the same lines, including familiar characters, and just as fantastic. I highly recommend it when you get the chance.
Not an entire game, but the scenario "USA 2010" from the late '90s for Civilization 2 may be kind of wat now. (Civilization II: Fantastic Worlds/USA 2010 — StrategyWiki) In 2010 at the start of scenario, there's already casual use of space stations. And force fields are a thing? (also the sprite art may not be that good)
